Population health
Involves looking at different factors that impact people's health, such as their social lives, the economy, and the environment.
Tracking hospital discharges
Involves documenting the number and specifics of patients who receive medical treatment before being released from a hospital.
Medication reconciliation
Involves creating an accurate and up-to-date list of all the medications a patient is taking to resolve any discrepancies that may arise during transition of care.
Patient self-management
Involves patients managing their own health and medical conditions through education, resources, and tools for health literacy.
Smoking status
Involves recording an individual's current or past smoking habits and their relationship with tobacco use to inform treatment decisions.
Counseling
Involves a trained professional who oversees the overall mental health and wellbeing of individuals, couples, families, or groups.
Training also includes proactive, preventative services such as Medicare Wellness Visits for Medicare Beneficiaries, which provide a snapshot of a patient's current health as well as a valuable baseline for future yearly visits. Medicare Wellness Visits are essential to ensure that any potentially serious health issues are identified as early as possible.
EVMS Medical Group belongs to the Sentara Quality Care Network (SQCN), an integrated care network led by forward-thinking healthcare providers.
